INSTRUCTIONS FOR THE APPLICANT:
To determine your insurability, a brief medical examination is required
as a part of your application. Your agent has chosen NMS Management
Services to perform your exam. The exam is usually completed in
20-30 minutes.
A Paramedical exam includes questions about your medical history,
height, weight, blood pressure and pulse. A urine specimen is also
collected.
Our examiner will ask the names and addresses of physicians and/or
hospitals. Our examiner will also ask about illness, surgery, medications
or other treatments you may have had.
Please be assured that all information obtained during the examination
will be held in the utmost confidence and will only be made available
to your insurance company. All information given to the paramedical
examiner is confidential and forwarded to the home office of the
insurance company and is for insurance underwriting purposes only.
What To Expect?
1. One of NMS Management Services examiners will
contact you to set up an appointment... at your convenience. You
may choose to have the examination performed at your home, your
place of business, or in our office.
2. Depending on your Insurance Company's requirements,
your examination may include any of the tests shown below. When
the examiner calls you for the appointment, you will be notified
which tests are required and approximately how long they should
take. The examination is performed at NO cost to you.
3. You will be informed by an Agent, or your examiner,
if additional requirements are needed such as an examination by
a licensed MD., EKG, Treadmill, Pulmonary Function Test, Blood Test
or a Chest X-Ray. You will be given the necessary instructions regarding
the tests required.
Applicant Preparation
and Information about the Tests
- Taking of Medical History
- A complete medical history will be taken including height,
weight, blood pressure and pulse.
- Urine Test:
- Drink water before the test to be able to provide a urine
specimen.
- Avoid vigorous exercise for 24 hours prior to the test
- Avoid alcoholic beverages for at least eight hours before
the exam.
- You will be required to sign an authorization form permitting
the examiner to obtain your urine and allowing the lab to
test it for specific tests
- Blood Tests: The blood specimen, as required
by your Insurance Company, may be taken from either your arm or
your finger. Please be assured that only sterile, disposable needles
will be used.
- Venipuncture Test: drawing blood from the arm
- Fingerstick Test: drawing blood by pricking the finger
- Do not eat for 4 to 12 hours depending on requirements hours
prior to the exam in preparation for your blood sample.
- Avoid vigorous exercise for 24 hours
- You may drink water, black coffee or tea with no sweeteners.
- A sterile blood kit authorized by the insurance company
will be used.
- Blood tests are performed by experienced and certified medical
examiners
- You will be required to sign an authorization form permitting
the examiner to obtain your blood and allowing the lab to
test it for specific tests
- EKG Test
- This electrocardiogram is a painless test that will require
you to partially disrobe, so privacy will be required.
- You are to have a private place to lie down.
- X-rays and/or Treadmill EKG
- Should these tests be required they are to be performed
at an approved doctor's office.
- All Tests:
- Avoid smoking, stress and exercise for minimally 2 hours
prior to the exam. Each exam may have more specific requirements
- Avoid alcohol and nasal decongestants for 2 hours prior
to the exam. Each exam may have more specific requirements.
- Try to relax prior to the exam for best results.
- You may be required to fast for a few hours prior to the
exam; however, water consumption is permitted.
- Your test results are forwarded to the insurance company
underwriting department by the lab.
- Requirements of applicant at time of exam
- A picture I.D., preferably a driver's license, available.
- All doctors' names and addresses available along with dates,
purpose and outcome of past visits
- A list of prescribed medications currently being taken.
Include the name, dosage, frequency and reason for the prescription.
